If you are covered by Community HealthChoices (CHC), they may be able to provide housing resources and guidance.

What is CHC?

If you’re 21 or older and have both Medicare and Medicaid, or receive long-term supports through Medicaid because you need help with everyday personal tasks, you will be covered by Community HealthChoices (CHC).

How do I access housing resources through CHC?

  1. Find your CHC’s contact information here.
  2. Search their website for housing-related resources OR
  3. Call them and ask for a housing specialist.

What Types of Resources are Available?

There may be funds available for the following needs:

  • Security deposit for a rental unit if are in an emergency shelter
  • Assistance with furnishings related to certain health conditions
  • Assistance with modifications due to a disability
